Survivor Revival Services Policies

CONFIDENTIALITY
Our utmost concern is the safety of those who use our facility and for those who use any services we offer. This office maintains the confidentiality of all business, and our Participants are expected to as well. This includes, but is not limited to, refraining from divulging the name or identity of other Participants who may use our facility or services, or divulging any information learned about other Participants while at our facility or during the course of using our services. Any confidentiality violations by Participants will result in removal from the premises and/or termination of the business relationship with you. 


TECH FREE SPACES POLICY
The Company has deliberately crafted its office space to foster intentional connection among all who enter. It firmly believes that technology, such as phones, tablets, and watches, can hinder genuine face-to-face human interaction. Consequently, the Company has implemented a policy that requires all electronics to be stored and muted in communal areas. In the playroom and sensory room, the use of electronic devices is strictly prohibited. To ensure the safety of personal devices while occupying the playroom or sensory room, the Company offers complimentary locking options for visitors to securely store their electronics during their visit. 


ROOM RENTAL AGREEMENT
All rooms available to rent on a 30-minute or 50-minute basis are utilized at your own discretion and own risk. Before using any room or any of the facilities, you will be required to read and sign an Assumption of Risk and Liability Waiver for yourself and your minor child.


PLAY AND SENSORY ROOMS
The designated rooms cater specifically to one parent and one child. It's important to note that there won't be any staff present during your scheduled time to facilitate interaction between the parent and child. Before entering the play room or sensory room, it is mandatory to sign an Assumption of Risk and Waiver of Liability. Additionally, adults must be present for the entire duration of their reservation.


Reservations for these rooms are allocated on a first come, first served basis. Please be aware that bringing food and drinks into these areas is not permitted, with the exception of water bottles, which can be stored in the cabinet outside the room. If you have any special needs or requests, we kindly ask that you reach out to us as soon as possible so that we may assist you to the best of our abilities.


All Participants are expected and required to read and understand any and all rules posted on any sign or in any handout provided to you. Failure to observe the rules may subject the Participant to removal from the premises and/or denial of any future services.


RESPITE/BEREAVEMENT ROOM 
For creative processing and restoration, it includes a large couch, sand tray, a shared infant loss grief journal, and a drawing table. Created for those experiencing infant loss, this space offers a retreat for adults needing respite and rejuvenation. Certain images and decor might cause distress.


NO WEAPONS POLICY
For the safety and well-being of everyone at The Company, we have implemented a strict no weapons policy within our facility. This policy applies to all individuals, and includes both concealed and open carry of any type of weapon. Our primary goal is to ensure a safe and secure environment for all Participants.


By prohibiting the possession of weapons, we are taking a proactive approach to minimize the risks associated with intentional or accidental use of such items. It is important to note that this policy applies regardless of whether the possessor holds a federal or state license to carry a weapon. If anyone is found to be in possession of a weapon, they will be kindly asked to remove it from our premises immediately.


We do, however, make an exception for law enforcement officers who are on duty. We understand that they have a responsibility to protect and serve, and therefore they are permitted to carry their weapons while in our facility.

I, the undersigned, agree to adhere to these policies around all room rental services. I take full responsibility for myself and all other minors who may be in my care at the time of rendered services. 


I understand that my failure to comply with these policies may result in a temporary or permanent prohibition of services at Survivor Revival Services. 

I also understand that Survivor Revival Services dba Facing Giants, is a small business committed to doing good in the world, and reserve the right to refuse service to anyone whose actions or intentions conflict with our values.
